Serving the counties of Ford, Iroquois, Livingston, Grundy, Will and Kankakee, UpliftedCare offers services for those struggling with serious illness with routine care and 24/7 on-call support. The UpliftedCare Community Grief Center offers grief support counseling, support groups and workshops for anyone experiencing grief even if their loved one did not receive care from UpliftedCare.
